In this Notice, LHA Mortgage Services, Inc. is referred to as we, our and us.

This initial privacy notice is for you if you are an individual (not a business) who has applied for and/or obtained a mortgage loan from us.

“Affiliate” means (1) any company that we own or control, plus (2) any company that owns or controls us, plus (3) any company that is owned or controlled by the same company that owns or controls us.

“Consumer Report Information” means information about you other than (1) your contact information (e.g., name and address), and (2) facts about your transactions and experiences with us (e.g., your loan amount). Examples of Consumer Report Information include your credit score and income.

“Nonpublic Personal Information” means any information that you provide to us to obtain a financial product or service, that we collect based on a transaction between us or that we obtain in other ways in connection with providing a financial product or service.

“Nonaffiliated Third Party” means a company that is not an Affiliate.

2. Information that We Collect

We collect Nonpublic Personal Information about you from the following sources:
• Information we receive from you on applications or other forms;
• Information about your transactions with us, and
• Information we receive from consumer reporting agencies and other third parties.

3. Confidentiality and Security of Your Nonpublic Personal Information

We maintain physical, electronic, and procedural safeguards that comply with federal standards to guard your Nonpublic Personal Information. We limit employee access to Nonpublic Personal Information to those with a business reason for accessing such information.

4. Categories of Information that We Disclose To Affiliates and Nonaffiliated Third Parties

We transmit Nonpublic Personal Information to investors that originate, purchase or service your mortgage loan to enable the investors to issue approvals and make underwriting/purchasing decisions. For example, if we transfer or sell your loan, we may provide your file, including your loan application, credit report and other loan documents to the investor/servicer. We also transmit Nonpublic Personal Information to credit reporting agencies, appraisers and other third parties as necessary to process your transaction. We also disclose Nonpublic Personal Information as necessary to prevent fraud or to comply with applicable state and/or federal law, for example, to comply with a subpoena, court order or regulatory demand.
• We reserve the right to disclose Nonpublic Personal Information to companies that perform marketing services on our behalf or to other financial institutions with whom we have joint marketing agreements.

In addition, unless you opt-out as described below, we may disclose Nonpublic Personal Information to Nonaffiliated Third Parties that provide insurance and investment products. These parties may inform you about insurance and investment products and services that may be useful, helpful and convenient to you. This enables us to better meet your financial needs.

You have the right to opt-out of our disclosure of Nonpublic Personal Information to Nonaffiliated Third Parties that provide insurance and investment products.

You also have the right to opt-out of our disclosure of Consumer Report Information to any Affiliate.

5. Explanation of Opt-Out Right

You may instruct us to refrain from sharing information, as described above, by opting out. If you choose to opt-out, we will not share your Nonpublic Personal Information with Nonaffiliated Third Parties except as provided by law nor will we share Consumer Report Information with our Affiliates. You may instruct us of your decision to opt-out by contacting us and advising our representative of your name, address, phone number and decision to opt-out or you may e-mail us at info@lhamortgage.com Upon receipt of your call or your e-mail exercising your opt-out rights, we will act upon it within a reasonable time and your opting-out will remain in effect until you notify us otherwise in writing.

Upon receipt of this notice, we will provide you a reasonable 30-day time period in which to exercise your opt-out rights.

6. Former Customers

After you are no longer a customer of LHA Mortgage Services, Inc. we will continue to treat the Nonpublic Personal Information we have about you as described in this Notice.
